Rochester agency expands its reach

Finger Lakes Performing Provider System is collaborating with Community Action for Wyoming to improve healthcare access for vulnerable and underserved individuals in the region.

FLPPS expands into Wyoming CountyCommunity Action will establish a telehealth hub at its location in Perry and its care coordinators will engage individuals and connect them to primary care and behavioral health services.

Spectrum Health Services will support the care coordinators by providing behavioral health intervention and peer counselor training. The care coordinators will provide peer counseling, referrals, navigation support and assistance with telehealth services.

As part of this collaboration, FLPPS is providing professional support to ensure the program meets its goals and helps those most in need to access healthcare services. This support includes consultation on data and analytics and a program evaluation plan.

“FLPPS is pleased to partner with an organization that understands the needs of their community and can directly address health disparities with appropriate access to care and resources,” said Carol Tegas, FLPPS executive director.
