Deeds of sale and mortgage documents were filed with the Monroe County Clerk on Friday, the final procedural steps in the transfer of ownership and operation of two area senior living facilities.

Post Acute Partners of New York City acquired six facilities across the state from Brentwood, Tenn.,-based Brookdale Senior Living, including Brookdale West Side on Long Pond Road in Greece and Brookdale East Side on Chardonnay Drive in Perinton.

Elderwood senior care facilities are a branch of Post Acute Partners. Elderwood assumed operational control of its new communities—renamed Elderwood Village at Greece and Elderwood Village at Fairport—on March 28.

Also included in the transaction were Elderwood Village at Bassett Park and Elderwood Village at St. Gregory Court in the Buffalo suburb of Williamsville, Elderwood Village at Vestal near Binghamton, and Elderwood Village at Colonie near Albany.

Elderwood’s portfolio now includes 15 assisted living/adult homes, 18 skilled nursing and two independent living facilities in the Northeast, with most in New York.

The new firm extended offers to more than 250 staff members in the six living communities.

Terms of the acquisition were not revealed. The sale price of the Elderwood Village at Fairport property was $3.5 million, and $2.5 million for the Elderwood Village at Greece property, records show. A total mortgage through M&T Bank worth $25.5 million was issued for the six facilities.
