NY Connects provides telephone guidance, screening for long-term care needs and preferences, and links to services. It is not a care management program, but it is often the best starting point because it provides direction to several long-term care services, including care management. 325-2800, www.nyconnects.ny.gov

Lifespan of Greater Rochester provides a variety of helpful services for older adults and caregivers in need of unbiased information, guidance and advocacy. 244-8400, www.lifespan-roch.org

Eldersource, a Lifespan and Catholic Family Center service, provides local care management services and caregiver consulting. Eldersource is a more intense service intended to support wellness screening, referrals and short-term guidance to individuals and families who need more thorough direction and assistance to connect to helpful services. 325-2800, www.eldersource.org

The Regional Center for Independent Living provides a variety of services, guidance and assistance to people with disabilities of any age and those who care for them. 442-6470, www.rcil.org

The Jewish Family Service of Rochester offers programs and services that support adult children of aging parents and provides care managements to elders at home. 461-0110, www.jfsrochester.org
