URMC wins $4.65 million grant

The University of Rochester Medical Center has won a $4.65 million grant to fund enhanced training of medical residents in patient-centered medical home physician offices.

In the medical home model, care managers based in primary care physicians’ offices work with doctors to coordinate care for healthy and chronically ill patients. Several medical home demonstration projects have been underway locally since 2009. Last year, Excellus BlueCross BlueShield said it had found seven medical home practices it sponsored to have achieved many of their treatment aims.

Awarded by the state Department of Health, the URMC grant originates from a $250 million Centers for Medicare and Medicaid fund to help medical schools give residents medical home experience.

The grant would allow URMC primary care residents to spend more time with Medicaid patients at its Strong Internal Medicine, Highland Family Medicine, Golisano Children’s Hospital Pediatric Practice and Culver Medical Group practice groups, URMC officials said.
